Capturing Marketing Consent on a Custom Intake Form

Use a custom intake form question to record whether a patient opts in to marketing from your brand.

Set up the consent question

  1. Open the form in Command's intake form builder.
  2. Add a Yes/No question.
  3. In the question properties, turn on Marketing consent question.
  4. Publish the form.

Only one question per form can be the marketing consent question, and only Yes/No questions are supported. When the patient answers Yes, the platform records consent for your affiliate; answering No (or changing a previous Yes answer to No) clears it.

Receive the answer

Subscribe to the custom_intake_form_completed webhook. The payload includes:

  • patient_id — the patient linked to the form instance (present on the webhook you receive; pre-login form completion does not trigger the event until after login/attach).
  • patient_marketing_consent — true if the patient answered Yes to your consent question, false otherwise.
  • responses — the full answer snapshot, including the consent question's location_id, label, and value.

The patient_marketing_consent value is captured when the event is created and does not change on retry. You receive one completion webhook per form instance, not a separate anonymous delivery followed by a second event after login.

Per-affiliate scope

Consent is stored per affiliate. A patient who opts in on Brand A's form will show patient_marketing_consent: false on Brand B's webhooks unless they also answer Yes on Brand B's form.
